I've noticed some yamaha outboards come with standard controls, meaning a simple shift/throttle lever with button at bottom of lever which, when engaged, allows for revving the motor in neutral.
other motors have a secondary lever with the ignition key put right in the control box itself. I assume this secondary lever is for neutral revving. What is the difference between the 2 and when is one style used over the other? Is their a difference in cost between the 2?
I have only ever used the first style mentioned and think that they look better, being simple and more compact.

I will give you my opinion to you question. I bought a boat and I had it rigged from the factory with Yamaha controls. I have a 225hp and a T-8 8 horse and the control is a single lever just like the big motor. If you bought the boat and did not have it rigged from the facorty for a kicker the dealer would probably put in the control box with the the two levers and the ignition switch built in which would be a lot easier to install then the single lever. They work about the same.
